Digital copies of Stanford’s requisitions can be found on the Test Requisitions page.
For clients who regularly send cases to Stanford, we provide pre-printed requisitions with your account information. To request a personalized req for your organization, please email ClinOpsContracts@stanfordhealthcare.org.
Stanford provides specimen collection supplies for established clients. In addition to individual supplies, we also offer collection kits for renal pathology, muscle and nerve pathology and other esoteric testing that may require non-standard materials for collection. Please contact ClinOpsContracts@stanfordhealthcare.org to enquire about specimens.
Stanford offers courier services to transport specimens within a specified region of the San Francisco Bay Area. These services will be set up at the time of account establishment.
For established clients outside of the San Francisco Bay Area, Stanford provides pre-paid FedEx shipping labels for the return of samples to our labs. We also offer regular and refrigerated shipping containers when appropriate.
Yes, Stanford offers third party billing for these services. If a pre-authorization for the services is required by the patient’s insurance, the referring facility is responsible for obtaining that authorization. If charges are denied due to lack of authorization or for any other reason, Stanford will client bill the facility any outstanding charges.
Stanford does accept limited healthcare insurance outside the state of California. That said, use of Stanford services by out of state patients often requires pre-authorization. We recommend confirming coverage directly with the patient’s insurance company.
Yes, Stanford can bill for cases that aren’t patient specific. Please contact our New client services on the Laboratory Testing Intake Form to discuss your specific interest and the representative can confirm that the specific work can be done.
